This guide helps you create a working video form, launch it, and collect your first response without setup friction. It then gives you a starting setup for four common workflows: candidate pre-screening, customer testimonials, product research with screen recording, and short mobile-friendly feedback forms.

Before you start

  • Define one clear goal for the form, for example candidate pre-screening, customer feedback, or product research.
  • Keep the first version short.
  • Plan for browser-based participation. Respondents can answer without installing an app or creating an account.

Create and launch your form

  1. Create a new video form. Give it a clear, action-focused title.
  2. Add your first questions. Start with open-ended prompts to capture richer context.
  3. Enable reply modes. Choose Video, Voice, Screen Recording, and Text as needed. See Choose Reply Modes and Recording Settings.
  4. Add conditional logic if you need it. Route respondents to follow-up questions based on their answers.
  5. Apply branding basics. Update the logo, colors, and thank-you screen.
  6. Run one internal test response. Check the full flow on desktop and mobile before you share the form.
  7. Share the form. See Share Your Video Form, then monitor early responses for quality and completion.

  • Are the reply modes matched to your audience?
  • Did a full test submission complete successfully?
  • Is your distribution channel ready: link, embed, email, Slack, or QR code?

Recommended setups by use case

Candidate pre-screening

For hiring and onboarding, enable Video + Voice with Text as a fallback. Video gives you emotion, tone, and richer context, while Voice and Text keep the form open to candidates who prefer not to appear on camera.

Customer testimonials

Video Recording is best for emotion, tone, and richer qualitative context, which is what a testimonial needs. Keep a Text fallback so customers who won’t record can still share their experience.

Product research with screen recording

For product feedback and bug reporting, enable Screen Recording + Voice with Text as a fallback. Screen Recording captures product walkthroughs and issue reports. Always enable a fallback when you ask for screen-based feedback.

Short mobile-friendly feedback form

For fast pulse checks, enable Voice + Text. Keep the form short and focused, and validate mobile behavior on real devices before rollout. A QR code works well for in-person touchpoints.

Common mistakes

  • Starting with too many questions.
  • Forcing one reply mode on every respondent.
  • Skipping internal testing before launch.
  • Using one setup for every audience without adapting it.
